Dear Parents/Guardians:
There has been a change in the way that some of the beginning of the year forms will be submitted. Fillmore Central is working on simplifying and automating the forms you are asked to complete. This will be a gradual process throughout the school year. Forms will be converted, if possible, with the goal of having most, if not all, forms converted by the end of the school year. This does not mean that ALL forms will be digital.
To start the process, you will be receiving two new forms in your child’s beginning of the year packet of papers. Please review, correct, and return them to your child’s building secretary. These forms have been partially completed for you with the information that is in PowerSchool. You just need to mark any changes, answer a few questions, sign and return. Hopefully this will shorten the amount of time needed for you to complete the forms.
The forms below can now be completed by family/household and submitted online. Please note that if you are unable or choose not to complete the form online, that hard copies are available in all three building offices.
